Definition: A legal document that allows you to specify who will make health care decisions for you if, for any reason, you can't make these decisions for yourself. These documents have different names in different states, such as a Medical or Health Care Power of Attorney, a Designation of Health Care Surrogate, or a Durable Power of Attorney for Health Care.
