Definition: An Advance Medical Directive is a type of legal document that allows you to specify who will make health care decisions for you if, for any reason, you cannot make these decisions for yourself. These documents have different names in different states, such as simply an Advance Directive, a Medical or Health Care Power of Attorney, a Designation of Health Care Surrogate, or a Durable Power of Attorney for Health Care.
